What is Targeted Youth Case Management?

Welcome to Legacy Education and Behavioral Health. We are honored to partner with your family as you navigate your child’s behavioral health journey.

Targeted Youth Case Management (TYCM) is a voluntary service designed to help children, adolescents, and their families access the supports and resources they need to thrive at home, in school, and in the community. Rather than providing counseling or therapy, your Targeted Youth Case Manager serves as a coordinator, advocate, and resource specialist who works alongside your family to connect services, reduce barriers, and help your child achieve their goals.

Every family is unique. Our services are individualized, family-driven, culturally responsive, and focused on building strengths while addressing challenges.


What Does a Targeted Youth Case Manager Do?

Your Case Manager will partner with you to:

  • Learn about your child’s strengths, needs, and goals.
  • Help identify barriers affecting your child’s success at home, school, or in the community.
  • Connect your family with behavioral health, medical, educational, and community resources.
  • Coordinate communication among providers, schools, and other involved agencies (with your written permission).
  • Advocate for your child and family to ensure your needs are understood and addressed.
  • Help develop an individualized service plan based on your family’s priorities.
  • Monitor progress and adjust services as your child’s needs change.
  • Support transition planning during changes in services, schools, or other significant life events.
  • Assist with crisis planning and connecting to appropriate crisis resources when needed.

What to Expect

Getting Started

Your first appointment is called an intake appointment. During this meeting, your Case Manager will:

  • Explain Targeted Youth Case Management services.
  • Review consent forms and answer any questions.
  • Learn about your child’s history, strengths, needs, and current supports.
  • Discuss your family’s goals and priorities.
  • Begin identifying services and resources that may be helpful.

This appointment usually lasts 60–90 minutes.


Developing a Service Plan

After the intake process, your Case Manager will work with you to develop an Individualized Service Plan. This plan outlines:

  • Your child’s strengths.
  • Areas where additional support is needed.
  • Family-identified goals.
  • Services and resources to be coordinated.
  • Specific action steps to help achieve your goals.

Your family’s voice is central to this process. Goals are developed together and reviewed regularly.


Ongoing Services

Services may include:

  • Meeting with your family in the office, at school, in the community, or virtually (when appropriate).
  • Coordinating care with therapists, medical providers, schools, and community agencies.
  • Helping complete applications for community resources and benefits.
  • Attending school meetings, such as IEP or 504 meetings, with your permission.
  • Identifying recreational, educational, and community opportunities.
  • Monitoring progress toward goals.
  • Updating your service plan as your child’s needs change.

The frequency of contact depends on your child’s individual needs and the goals identified in the service plan.


Your Role

Families are the experts on their children. Your participation is one of the most important factors in successful services. 

We encourage you to:

  • Attend scheduled appointments whenever possible.
  • Share changes in your child’s needs or circumstances.
  • Communicate openly with your Case Manager.
  • Participate in developing and reviewing your child’s service plan.
  • Notify us if your contact information changes.
  • Ask questions whenever something is unclear.

What Targeted Youth Case Management Does Not Include

Targeted Youth Case Management is not:

  • Individual, family, or group therapy.
  • Professional mentorship.
  • Psychological testing or evaluations.
  • Medication management.
  • Crisis response or emergency services.
  • Transportation services.
  • Legal representation.
  • Childcare or respite services.

If your child needs these services, your Case Manager can help connect you with appropriate providers and community resources.


Confidentiality

Your family’s privacy is important to us. Information about your child and family is kept confidential in accordance with federal and state privacy laws. We will only share information with other providers or agencies when you have signed the appropriate authorization, or when disclosure is required or permitted by law (such as concerns about abuse, neglect, or imminent safety risks).


Your Rights

As a participant in Targeted Youth Case Management services, you have the right to:

  • Be treated with dignity and respect.
  • Participate in all decisions regarding your child’s services.
  • Receive services that are family-driven and culturally responsive.
  • Ask questions and receive understandable information.
  • Request changes to your service plan.
  • Decline services or discontinue participation at any time.
  • Voice concerns or file a grievance without fear of retaliation.

Ike Wallace – TYCM

A Missoula native, Ike earned his B.S. in Education from Montana State University in 2014. He has over a decade of experience working with elementary, middle and high school students in formal/informal classroom settings, various coaching positions, and several non-profits roles across Montana and beyond. In his free time, Ike enjoys running, fishing, biking, skiing, skating, traveling, reading, listening to and making music. He’s often accompanied on his adventures by his dog Vinnie.
